04/09/2019 D-HAUE(3) (photo) Aerospatiale 350B3 Ecureuil (3549) --- Aerospatiale 350B3 Ecureuil D-HAUE(3) crashed at around 10:45 a.m. local time on take-off from Pampilhosa da Serra Airfield, Coimbra, Portugal, whilst on fire-fighting duty. The accident injured a minor, a member of the Grupo de Intervencao de Protecao e Socorro (GIPS) who was travelling in the helicopter to help fight the fire in Proenca-a-Nova. On the helicopter were six people, the pilot and five members of the GIPS-team. In a press conference, the Minister of Internal Affairs, Eduardo Cabrita, revealed that the accident occured due to a mechanical braekdown. According to the pilot's statements, the engine start-up and flight preparation procedures were normal with no relevant issues to record until the time the Flight Command was selected, and a sudden increase in engine power was noticed. Immediately after, the aircraft began a vertical movement with some oscillations. The aircraft rotated 90º clockwise, rose from the ground to a height of about one meter and the pilot could not counteract the low tail attitude and the descent aircraft movement. On the downward trajectory, the aircraft touched hard with the back right leg and the tail rotor strike on the ground was inevitable. The hlicopter returned to the air and rotated around its longitudinal axis until the main rotor also touched the ground. The aircraft without tail rotor and with the main rotor severely damaged became immobilized on its right side, about 3 meters ahead of the initial touch with the leg and tail rotor. The investigation establishes as probable cause for the event the inadvertent movement of the twist grip by the pilot to the emergency range, MAX. For the pilot actions on the aircraft controls may have contributed the a) the different AS350 B3 2B and 2B1 models’ configurations; b) grip modification (073084) was not embodied on aircraft; c) pilot recent flight experience on the 2B1 model and the lack of recognition on the 2B1 to 2B models differences; d) no onboard checklist adherence. D-HCOL (photo) Aerospatiale AS350B2 Ecureuil (2814) / y/b 1994 --- AS350B2 Ecureuil c/n 2814 suffered an aerial electrical cable collision following water discharge during fire-fighting operations at S. Martinho das Amoreiras, municipality of Odemira, Portugal. After the water discharge the pilot, through his peripheral vision, realized the proximity of electrical cables below the helicopter and immediately carried out an evasive maneuver with a sharp nose up attitude. This maneuver was not enough to prevent the clash of the bucket in the electrical cables. The pilot felt the impact and, through the rearview mirror, saw the bucket oscillating and rising toward the helicopter tail. The pilot instinctively actuated the bucket manual release but it ended up curling and got stuck in the antennas located in the helicopter tail. A member of the heliborne brigade on the ground confirmed to the pilot that the bucket was curled and stuck in the helicopter tail. Given that the helicopter had positive maneuverability the pilot immediately landed about 300 meters ahead from the impact site. . Initial assessment reveals bucket, helicopter tail and one main rotor blade damage. Pilot o.k. (dmg)

D-HCKV (photo) Agusta A109A (7345) --- While conducting a VFR flight from Blackpool to a hotel landing site at Newby Bridge, Cumbria, United Kingdom, the pilot approached the estuary off Morecambe Bay, about 8nm from his destination and could see what appeared to be a small shower in the area of the destination. Before turning east he could see that the visibility was going to deteriorate in the shower ahead and he decended further to 500 feet and reduced speed to 80 kt IAS, having the intention to remain well below the cloud base. As the pilot turned east into the mouth of the valley he flew into light drizzle which reduced the visibility to some 4kms while the general overcast and poor visibilty produced a white 'goldfish bowl' condition with no horizon. During a right turn in the valley the pilot noticed that the white misty area ahead suddenly turned dark and then he saw trees about 50 yards away and level. He applied maximum torque and pitched the helicopter nose up but the aircraft struck the trees in a flared attitude. The impact caused severe damage to the main and tail rotor blades including the loss of both tail rotor blade tips. On three of the four main rotor blades part of the blade tips were lost. One main rotor blade spar was bent upwards, and a substantial part of the lower tail fin had been removed. The right side stabiliser was lost and there was damage to the fuselage. The helicopter was vibrating and shaking badly with very low forward speed. He maintained maximum torque and as little speed as possible in order to obtain the best climb gradient to avoid the terrain ahead. With a rate of climb of approx. 1000 feet/min directional control was very difficult and the vibration was extreme. At 800 feet radio altimeter he reduced torque and accelerated to 80kt which made the helicopter much easier to handle. At 50% torque the vibration was manageable. The helicopter quickly broke cloud and the pilot saw Cark Airfield about 4nm away. The ground below was waterlogged precluding an immediate run on landing. A vertical landing would have required high torque which would have increased the stress on the already damaged blades and aircraft structure. He approached the into wind runway at Cark Airfield, maintaining almost constant 30% torque, and touching down at 40kt. The helicopter was shutdown and all occupants exited through the normal doors. After an injured boy has been cared for, the helicopter took off with the child on board but unfortunately hit the highway bridge with its tailrotor, causing the helicopter to crash and burst into flames. The injured person to be rescued was a 9-year-old boy who later died at Würzburg hospital. An initiated criminal case against the pilot for negligent manslaughter has been discontinued as forensic medicine found, that the impact with the helicopter was unrelated to the boy's death. The injuries sustained in the car accident would have led to death even without the use of a helicopter. (1k., w/o)

17/08/1975 D-HAST (photo) Bell 206A JetRanger (504) --- The helicopter was used by Cpt. Barry Meeker to enable a family of four to flee across the Iron Curtain. On 17th August 1975 he was to pick up a family of four on the banks of the Vltava River, Czechoslovakia, but the escape-willing came a few moments too late. Instead of ten seconds, the helicopter hovers for nearly two minutes at the meeting point. Cpt. Meeker lets his friend Teddy K., a flight enthusiastic student from Marburg, get off the helicopter to help the family getting on board. Front soldiers alerted by the turbine noise open up fire to the JetRanger, hitting the pilot on the arm. He leaves Teddy K. and the wife of the family behind, as the woman collapses hit by a bullet. The helicopter was flown by wounded Cpt. Meeker to Traunstein and landed safely at the hospital with numerous bullet holes. (1k., s-dmg)

06/09/1972 D-HAQU Dornier-Bell UH-1D Iroquois (8060) --- UH-1D of Bundesgrenzschutz was blown up at Fürstenfeldbruck Air Base by Black September terrorits who held nine Israeli hostages. Around 10:30 p.m., the terrorists and hostages were transported to Fürstenfeldbruck airport by helicopter. Once on the ground, the terrorists realized there was a trap. Snipers started shooting at them and they shot back. Two terrorists and one policeman were killed. Then a stalemate developed. Officials requested armored cars and waited for over an hour for them to arrive. When the armored cars arrived, the terrorists knew the end had come. One of the terrorists jumped into a helicopter and shot four of the hostages, then threw a grenade into Dornier-Bell UH-1D D-HAQO (c/n 8059). Another terrorist hopped into Dornier-Bell UH-1D D-HAQU (c/n 8060) and used his machine gun to kill the remaining five hostages. The snipers and armored cars killed three more terrorists in this second round of gunfire. Three terrorists survived the attack and were taken into custody. (w/o)

17/08/1971 D-HILF (photo) MBB BO105A (S-5) --- MBB BO105 operated as "Christoph 1" crashed at München/Allach, Oberbayern, Bavaria, S Germany, while on an EMS mission. On this day, the recently established air ambulance service suffered a sad setback. At around 8:53 a.m. "Christoph 1" was called to a traffic accident in which a boy was seriously injured. A few minutes later, the EMS helicopter approached the MAN company sports field at München/Allach. The accident occurred when the tail rotor grazed the meadow of the sports field, whereby the tail rotor shaft broke, causing the helicopter to spin around due to lack of torque compensation. Reflectively, the pilot pulled the BO105 up and while the helicopter came back to the ground, turning on its own axis, the doctor, Dr. med. Joachim Guskar, was thrown out of the machine and died at the scene of the accident. Unfortunately, the doctor had already loosened his seat belt on the approach and opened the sliding door to get out. The 24-year-old paramedic Joachim Schulze survived the accident with serious but not life-threatening injuries. The pilot Ulrich Trampnau got away with just minor injuries. 00/00/1964 D-HODE (photo) Westland-Sikorsky 55 Whirlwind Srs. 3 (WA402) --- Capt. Willy Weitzel had an engine failure close to the rig Mr. Louie. He landed the Whirlwind perfectly on the sea, and the helicopter was manoeuvred to the rig and hoisted alongside from its lifting eye by the rig crane. It was suspended there for most of the day until a rig supply boat came alongside and the helicopter was lowered on to it without a scratch. The supply boat then returned to Emden docks where a huge dockside crane was positioned overhead and the helicopter attached, still by its lifting eye. The crane operator was positioned way up in the clouds and when signalled to raise the helicopter, it was snatched off the boat rapidly to height of about 30 ft and then the lifting eye unscrewed itself and the helicopter did its last hover before gently descending back on the boat with a crunch.
